1.3 Evaluation and distribution\u000DThis document is a second version of the rehabilitation protocol, first drawn up in 2010. The past few years can be seen as a continuous process of evaluation. Practical procedures, guidelines for multidisciplinary team meetings, use of the ICF model and refinement of the scientific evidence for the programme have been a constant focus of attention. The team aims to offer an optimal SMART (specific, measurable, acceptable, realistic, and time-based) rehabilitation programme, in which the outcomes will be continually evaluated and improved. It was already stated in version 1.0 of this programme that it would frequently be updated to see whether any improvements should/could be implemented. Furthermore, it is essential to incorporate the latest research/developments within head and neck oncology and related disciplines in the HNR treatment programme.\u000DImportant alterations made in this version (version 2.0) are:\u000D- A specialised case manager has been added to the treatment team (e.g. Chapter 2 and 3).\u000D- The scientific evidence in Chapter 5 now includes the latest research. The scientific evidence for\u000Dphysical therapy and occupational therapy now are described separately, and activity guidance\u000Dand creative therapy have been added to the paragraph on psychosocial guidance.\u000D- Based on new insights and added experience, treatment modules have been modified where\u000Dnecessary (Appendix 1E).\u000D- The ICF codes have been added to the different treatment modules.\u000DVersion 2.0 of this programme is again available for health care professionals working within head and neck cancer institute\u2019s collaborating within the Dutch Head and Neck Society (NWHHT), and through this English version, now also for foreign colleagues, who would like to set up a rehabilitation programme in collaboration with a certified rehabilitation centre.\u000DIn the following chapters an overview will be given of the content, treatment process and Organization involved in the HNR treatment programme. Finally, the scientific evidence for the HNR programme per discipline will be discussed.\u000D85\u000D
