multidisciplinary rehabilitation by a consistent team of health professionals, who systematically inform each other of treatment progress and consult with each other in team meetings.\u000DThe rehabilitative care is in line with the head and neck cancer treatment protocols established at the NKI-AVL (www.hoofdhalskanker.info) and the rehabilitation care views of Reade. Where necessary, the rehabilitation programme will be employed during or after the oncological treatment. Furthermore, this structured HNR programme will be validated scientifically where relevant.\u000D2.6 Inclusion and exclusion criteria\u000DTable B offers an overview of the inclusion and exclusion criteria of the patient eligible for participating in the head and neck rehabilitation (HNR) programme.\u000DTable B In- and exclusion criteria HNR programme\u000D2.7 Timing and amount of information\u000DThe patient is informed using various forms of appropriate informational material (brochures (see appendix 5E), the Internet, presentations). Each member of the rehabilitation team can give more information on the possibilities for rehabilitation in general. Each member of the rehabilitation team involved with the rehabilitating patient will individually give out the necessary information concerning the content and the aims of rehabilitation within their own discipline. The case manager will be the contact person for the patient for the duration of the rehabilitation process, and the patient can approach her/him with questions at all times.\u000DInclusion criteria:\u000DExclusion criteria:\u000D\u2022 Patients with a tumour in the head and neck area who have been treated for this with surgery and/or radiotherapy with or without concomitant chemotherapy.\u000D\u2022 Patient is physically capable of taking part in a treatment programme.\u000D\u2022 Patient is mentally/psychologically capable of taking part in a treatment programme.\u000D\u2022 Patient is motivated to work together with health professionals in achieving the desired (SMART) outcomes.\u000D\u2022 Patient is teachable and trainable and understands the Dutch language.\u000D\u2022 Patients with interfering comorbidity.\u000D\u2022 Patients with psychiatric problems (e.g. an\u000Daddiction that would hinder the correct execution of the rehabilitation programme).\u000D89\u000D
