Along the way, these goals can be reviewed with the patient and, where relevant, adjusted or checked off the list.\u000DThe case manager is responsible for the logistic coordination of the rehabilitation and plays the role of contact person for the patient. Ultimately, a final evaluation takes place, which is to determine whether the rehabilitation goals and main problem have been successfully addressed. The case manager subsequently performs an overall quality-of-life assessment, along with a survey developed especially for the HNR programme (see Appendix 6E).\u000D3.1.5 Digital report\u000DThe reports of the team meetings are filed in the patient\u0027s electronic medical record (EMR). Standardised digital forms are available for this, and these are filled in by each separate team member prior to the meeting. In the meeting itself, an overview of the SMART goals, the main goal, the most recent information of the disciplines, and the report of the previous MDT meeting is projected on a large screen. During the MDT meeting, the points of discussion and possible adjustments of the defined goals will be documented by the case manager.\u000D92\u000D
