3.7 Communication\u000DThe rehabilitation process is a multidisciplinary affair, in which mutual agreement between the various disciplines is of vital importance. Agreements on the interdisciplinary exchange of information and decision-making throughout the rehabilitation process are frequently evaluated in the multidisciplinary team (MDT) meeting, whereby the goals and progress of the rehabilitation programme are systematically addressed using the SMART model. Working with the EMR, which all therapists can review and edit from their own workplaces, enables optimal communication and multidisciplinary collaboration.\u000D95\u000D
